Participant Waiver, Release & Consent
Applies to all For the Players Foundation programs and activities — including free pickup soccer, soccer teams, clinics, camps, and special events.
Please read this waiver carefully. It applies to your child's participation in any For the Players Foundation ("FTP") program or activity. The parent or legal guardian agrees to these terms on behalf of themselves and their child by signing or submitting a program registration or waiver form.
1. Permission to Participate
I am the parent or legal guardian of the participant, and I give permission for them to take part in FTP programs and activities. I confirm the information I provide at registration is accurate.
2. Acknowledgment & Assumption of Risk
I understand that soccer and related physical activities carry inherent risks — including running, contact, falls, weather exposure, contact with equipment or other participants, and the possibility of minor to serious injury. Knowing these risks, I voluntarily choose to have my child participate and accept the risks involved.
3. Release & Waiver of Liability
To the fullest extent permitted by law, I agree that neither I nor my child will hold For the Players Foundation, its staff, coaches, volunteers, board members, sponsors, or the owners and operators of any facility used for its programs responsible for injuries, losses, or damages arising from ordinary participation — except where caused by willful or reckless misconduct.
4. Medical Treatment Authorization
If my child is injured or becomes ill during a program and I cannot be reached, I authorize FTP staff and volunteers to seek and obtain emergency medical care for my child. I understand I am responsible for any resulting medical costs, and I will disclose any allergies, conditions, or medications at registration.
5. Photo & Media Release
I grant For the Players Foundation permission to take and use photographs and video of my child for promotional, educational, and fundraising purposes (including website, social media, and print), without compensation. I understand I may opt out at registration.
6. Code of Conduct
My child and our family will treat coaches, staff, volunteers, other players, and facilities with respect, and will follow program rules and staff instructions. I understand FTP may remove a participant for unsafe or disruptive behavior.
7. Drop-off, Pick-up & Supervision
I understand supervision is provided only during scheduled program times, and that I am responsible for my child's transportation and timely pick-up. For programs with a specific drop-off and pick-up procedure (such as a designated gate or numbered-tag system), I agree to follow it as directed by staff.
